Sacramental Requests
- For parishioners seeking infant baptism, please contact the office or download and complete the baptism form below.
- For information regarding weddings at Saint Boniface and the wedding reservation form, please visit our Weddings page.
- For parishioners in an emergency situation needing a priest, especially for last rites, please call the parish office at 502.584.4279. If the office is closed, you will be connected to an answering service which will notify the priest.
- For parishioners seeking other sacraments (anointing of the sick, confession, first communion, confirmation), RCIA, or a funeral, please contact the parish office by phone, email, or using the contact form at the bottom of this page.
Parish Registration
For parish sacraments, programs, and ministries, please register as a member of Saint Boniface by downloading and completing the parish registration form below. Paper copies are also available near the doors of the church. Registration forms can be submitted by email, postal mail, or by placing them in the collection basket during Mass.
